- Applicants should complete and sign the application form.
- Submit application to CMI along with $75 administrative application fee.
- Contact all of the references and give reference form. They should mail the reference forms to CMI at

- Submit a photo for the identification card.
- Follow-up with pastors and ministers to ensure that references have been submitted to CMI and confirm the delivery to CMI at the above address.
- Have three (3) recognized ordained ministers mail a reference letter to CMI. One of the
letters should be your Pastor. Please have these ministers send reference letters directly
to Covenant Ministries International.
